From what I could see from the on line product guide it looks like you have 4 screws you should have 1 silver screw and 3 brass colored screws , one side will prob. be the silver and a brass colored screw and the other side will be 2 brass colored screws with a piece of metal connecting the two, first thing you should do is break off the metal piece that connects them, next you need a piece of wire to jump out the switch part and outlet part, connect the wire to the brass screw on the side with the silver screw and then connect it to the brass screw on the other side where the plug section is located ( top or bottom) and then you will want to take the wires that you had on the plug usually black and white and put the white on the silver screw and the black on the last brass screw , If you have more then one set of wires in the box you will need to splice them together with a tail coming off it
